CA HOV Sticker Application When Leasing

Hello,
Just received my registration and plates and now in the process of filing the REG 1000 form to obtain the white ZEV decals for my 2015 Leaf. I was told by the dealer to put my name in True Full Name section "as it appears on the vehicle registration card.

However, since I am leasing the vehicle, the Registered Owner appears as follows on my vehicle registration:

Nissan Infiniti LT LSR
Shawn Johnson LSE

Just wondering if anyone else ran into this when filing out this form. Thanks in advance!!!

I just used my name only, no mention of NMAC (or other lessor if yours is not), and had no problems.

watch for when you check is cashed. a month is about right.
