I also am not clear. It could be due to some cars that do have a bug where the CCS plug doesn't unlock or user error (judging by these: https://www.youtube.com/watch?v=2l8iNJn42dc, https://www.e-tronforum.com/threads/charger-stuck.2846/#post-27732, https://www.audiworld.com/forums/audi-e-tron-232/got-trapped-fast-charger-wouldnt-release-2971959/, https://www.audiworld.com/forums/audi-e-tron-232/locked-charge-port-door-solution-another-terrible-holiday-trip-failure-audi-ea-2984096/).

Or, it could be people dropping the handles. Or, for some, it could be due to the layout and cables jutting out:

The above two sites have had CCS broken tangs multiple times. In the narrow spaces, the cables jut out really far unless you get out of your car and push them back first. I suspect that some people when parking or departing, their car rubs against them, when then pushes them forward/back from their holster, which could break the tang.

But, I've also seen broken tangs at some CCS sites that don't have this narrow constraint problem. For all I know, it they could be brittle or just people are tripping over the cables or both.

This style of EA Signet (the only style of EA Signet I've personally seen) at https://www.plugshare.com/location/296759 has the cables and holsters on the side, so there isn't the cables jutting out problem.
