I will be parking my 2018 Leaf S for the next five months. It will be parked in a garage that will be around 45 degrees F. Are there any particular issues I should be aware of to make sure I am taking proper care of the main (and 12V) batteries?
 
Nothing special that I'm aware of. I'd leave the main battery at 30-40% SOC and I'd also top off the 12V battery with an external charger. The latter probably isn't absolutely necessary, although I'd do it if isn't too much hassle for you.

I agree on both points, although if you need a bit more charge anything under 60% is fine.
