For the longest time, it was only Tesla that would. The Model S (and presumably the X) come with http://shop.teslamotors.com/products/model-s-mobile-connector-bundle, which is capable of up to 40 amps @ 240 volts.

I was surprised to learn the Volvo XC90 supposedly includes Turbocord, per http://insideevs.com/2016-volvo-xc90-t8-plug-hybrid-include-free-aerovironment-turbocord/ and many other sources.

Any others for the US market that provide a 208/240 volt 15+ amp EVSE w/the car?

Actually, there are 2 versions of TurboCord. One is 208/240 volts only, while the other (with Dual in the name) also can do 120 volts.
 
Although it is no longer in production, Honda included a Leviton L2 EVSE with every Fit EV it leased on or after June 1, 2013. From http://automobiles.honda.com/fit-ev/charging-an-electric-vehicle.aspx

Installing a home charging station is the best way to ensure that you have reliable access to a 240-volt circuit. Honda has selected Leviton as the Electric Vehicle Supply Equipment (EVSE) vendor that will provide the Fit EV home charging station, and as of June 1, 2013, Leviton’s home charging station hardware is included in your Fit EV lease. Leviton will obtain all the required permits from your local municipality before installing the system. Installation costs and permitting fees may be extra and may vary depending upon your home charging station needs, and these costs fall under the responsibility of the Honda Fit EV lessee. Also, when the 3-year lease is completed, the charging hardware will remain property of the lessee.
